• The self-determination program (SDP) allows individuals with an individual program plan
(IPP) the ability to freely choose the services and supports that help them meet the
goals described in their IPP.
This program provides individuals The right and responsibility to make decisions about:
• What support they want to help them achieve their goals.
• Who they want to provide that support.
Who can participate in the Self-Determination Program?
• All eligible individuals receiving services from a regional center.
• To obtain additional information about the SDP, please reach out to the regional center
that is currently providing services to you.
What is an independent facilitator?
• An independent facilitator is a person selected by the individual in the SDP who assists
the individual in making informed decisions about various aspects related to the SDP.
• ABS clinicians are trained as independent facilitators in the principals of: selfdetermination, person-centered planning, and other various components of the SDP
program (e.g., service provision, budget creation, advocacy, etc.)
What does an independent facilitator do?
• The independent facilitator can provide information about the individual’s budget, and
locating, obtaining, and scheduling services related to the IPP.
• The independent facilitator can also assist the individual by helping identify long-term
and short-term needs and creating a plan to address those needs.
• An independent facilitator may also lead or advocate for the individual in the creation of
the IPP.
What will we do?
• Provide you with all relevant information to:
- Create a budget for services and other important resources
- Advocate and help develop a person-centered plan that best fits your needs
- Identify, obtain, and schedule the appropriate services that you want to help you
meet your long and short-term goals.
• Provide long-term planning & support
- We can consult monthly or as needed to:
• Manage and track your budget
• Make changes spending plans
• Assist with the Financial Management Service (FMS)
• Track goals on the person-centered plan
• Provide caregiver, staff, and resource support through training, meetings,
and consultations
